STAR ACCOMPLISHMENTS Fourth Quarter 2006

Medicare Recertification Survey was done December 15th thru 18th showing our agency being in “substantial compliance” with the Conditions of Participation.

We had 3 stars for our Alabama Quality Assurance Foundation (AQAF) selected outcomes in December meaning our agency either met or exceeded the goals set by AQAF for our agency.

We showed improvement in 10 out of 10 outcomes on Medicare.gov from June 2006 to December 2006.

Evergreen Home Care was interviewed for the November issue of ‘Home Care Outcomes’ relating to our improvement in our Acute Care Hospitalization outcome.

Susan Harp, EHC Director, was appointed the President-Elect position for the Home Care Association of Alabama for 2007.

Evergreen Home Care is participating in the Home Health Quality Improvement National Campaign which is designed to allow agencies to network together as a national effort to reduce unnecessary hospitalizations.

Amy Ellis, RN, one of our nurses was awarded ‘Employee of the Month’ for December 2006.
