To familiarize you with the services and procedures of Evergreen Medical Center, we have prepared this convenient Guide to Guest Services. We hope this will answer most of the questions you may have. We want your stay at Evergreen Medical Center to be as pleasant as possible. If you have any questions or concerns, please don’t hesitate to ask your physician or your nurse. If at any time you feel you are not being treated in a fair and concerned manner, please notify Administration at Extension 160.

ABOUT EVERGREEN MEDICAL CENTER
Evergreen Medical Center is an affiliate of Gilliard Health Services, Inc. Our network of resources includes several hospitals in South Alabama, which brings together the skills of a vast array of highly trained and caring medical professionals, as well as the latest in state-of-the-art equipment and technology.

We are licensed and accredited by the state of Alabama to operate our 58-bed facility. We provide 24-hour a day emergency room services, laboratory, x-ray facilities, respiratory therapy, pharmaceutical services, surgery, rehabilitation services, psychiatric services and home health care.

YOUR ADMISSION
A member of Evergreen Medical Center’s medical staff requested your admission. Our admission personnel will obtain information from you that is required by law and will help us to serve you better.


YOUR ROOM
Private and semi-private rooms are available at Evergreen Medical Center. Your room assignment is based on the availability on the day of your admission. If you desire a transfer to another type of room, please make your request known to the nursing staff and they will do their best to accommodate you. Medical insurance usually does not cover the full cost of a private room. If yours does not, a $50.00 private room deposit will be required.

Television:
The Evergreen Medical Center Auxiliary provides the televisions equipped with basic cable. There is a $2.00 per day usage fee to be paid upon admission that is donated to the auxiliary for further enhancements to the Medical Center. Please refer to the Volunteer section for more information on the auxiliary. When using your television, please be considerate of other patients by turning volume down and by turning off your set at bedtime.

Telephones:
Your family and friends may call your room by dialing 578-0-room#. Callers who do not know your room number may reach you through the Medical Center’s main number at (251) 578 -2480.

• To access an outside line from your room: dial 9, wait for a dial tone, and then dial the number you are calling. There is no charge for local calls.
• To call long distance or collect: dial 0, ask the switch board operator for an outside operator
• To use a calling card: dial 9, wait for a dial tone, and then follow the instructions on your calling card.

If you have any problems with the Medical Center phone in your room, please do not hesitate to ask your nurse to get in touch with your Patient Representative.

Your Mail and Flowers:
A volunteer will deliver your mail to you twice a day. If you have a stamped, addressed letter you wish to mail, you may have someone drop it off at the front desk or give it to a volunteer or your Patient Representative. Mail received after you have been discharged will be forwarded to your home. Your flowers will be delivered to your room as soon as they arrive at the Medical Center.

Newspapers:
The City Drug Store, Inc provides The Evergreen Courant for you every Thursday.

Notary Public:
The services of a notary are available for patients free of charge. For more information, please ask your Nurse or Nurse Manager.


FOR FAMILY AND FRIENDS


Visiting Hours:
Visitors can be good medicine for patients, and we understand how important they are in your recovery. Your family and friends may visit you between the hours of 9:00 a.m. and 9:00 p.m. We do ask that visitors please follow the guidelines and regulations established by the Medical Center to insure your speedy recovery.

Visiting Regulations:
• Visitors may not smoke in patient rooms or anywhere in the hospital. There are designated smoking areas outside at the end of each corridor.
• Children under the age of 12 are welcome to visit parents, grandparents, and siblings – as long as an adult accompanies them.
• V isits should be kept short; and visitors should maintain a quiet environment and avoid unnecessary noise.
• The number of visitors is limited to 4 in private patient rooms, and 2 in semi-private rooms.
• Visitors may be asked to leave the room during tests or treatments or when the doctor or nurse needs to see the patient.

Parking:
Parking for patients and visitors is available directly in front of the hospital main entrance. There is additional parking in the lot adjacent to the Business Office located to the left of the main entrance to the hospital.

Guest Meals:
One guest tray per meal may be purchased for $3.50 from the Receptionist in the front lobby. If you would like to receive a meal for lunch, please notify the Receptionist before 10:00 a.m. on that day. If you wish to purchase dinner in the evening or breakfast for the following morning, please notify the Receptionist before 4:00 p.m. in the afternoon. If after 4:00 p.m., please notify Registration.

Chapel:
The chapel at Evergreen Medical Center is located near the nurse’s station. It is open continuously. Families and friends of patients, as well as clergy, are encouraged to use the chapel at their convenience.


FOR YOUR SAFETY AND SECURITY

Medications:
All medications you take while you are in the hospital are prescribed by your doctor, dispensed by the hospital pharmacy and administered by a nurse. Patients are not permitted to administer their own drugs or keep personal medications at their bedsides. Upon admission, please give all medications from home to your nurse, who will administer them to you.

Smoking:
Patients and visitors are not permitted to smoke in the Medical Center. There are designated smoking areas outside at the end of each corridor.

Fire Drills:
For your protection, the hospital conducts fire and disaster drills regularly. If a drill occurs while you are here, please remain in your room. The staff will notify you of any action that should be taken. Please be assured that our staff is fully trained in fire and disaster protection.

Valuable or Lost Items:
Patients are asked not to bring items of value to the hospital with them. If you do bring a valuable item, it should be given to the Nurse Supervisor for safekeeping. The hospital does not accept responsibility for items of value unless they are given to the Supervisor. If you do lose something, notify your nurse right away, and we will make every effort to help you find it. Unclaimed articles are turned in to the Nurse’s Station. To claim any lost and found items, please go to the Nurse’s Station.

Cell Phones/Personal Electronic Devices:
The use of personal electrical devices and cellular phones is strictly prohibited within the Medical Center.


YOUR HOSPITAL TEAM

The Medical Staff:
The physician who admits you is responsible for directing your care while you are a patient at the Medical Center. As the director of your care, your physician should be consulted if you have any questions about your illness.

The Nursing Staff:
A team of professional registered nurses, licensed practical nurses and patient care technicians provides quality nursing care. A nurse manager is responsible for coordinating care in each department. Please feel free to contact your nurse or nurse manager if you have questions or concerns. Nursing supervisors are on duty 24 hours a day and are available to assist you if the nurse manager is not available. If you encounter any problems that are not being resolved during your stay at Evergreen Medical Center, please call the Director of Nursing at extension 136 for further assistance.

Diagnostic Services:
Radiology at Evergreen Medical Center includes CT scanning, MRI, ultrasound and mammography. Board-certified radiologists assist Evergreen Medical Center by interpreting test results and providing your physician with the most accurate picture available to help diagnose illnesses.

Dietician:
A registered/licensed dietician is available to Evergreen Medical Center to meet your nutritional needs during your stay. If you have a question about your meals or diet, please ask your physician or your nurse.

Housekeeping:
Before you arrived, your room was thoroughly cleaned and inspected. It will be cleaned daily during your stay with us. If your room is not cleaned to your satisfaction, please notify Facilities Management at extension 139.

Pastoral Care:
A number of local pastors serve in Evergreen Medical Center’s volunteer, on-call chaplain program. A chaplain is available to assist you and your family at all times. You may contact a chaplain through your Patient Representative or Nurse.

Physical Therapy:
Physical therapy services are available Monday through Friday. Physical therapy seeks to prevent, identify, correct and alleviate movement problems related to injury, disease, post-surgery rehabilitation and individualized developmental abnormalities.

Evergreen Medical Center’s Rehabilitation Services Department provides physical therapy to inpatients, outpatients and our home health care patients upon physician referral. The scope of services includes sports injuries, wound and pain management, post-surgical rehabilitation, and geriatric and pediatric rehabilitation. Registered physical therapists and licensed physical therapist assistants provide these services.

Case Management:
A licensed, professional case manager acts as a liaison between the patient, family, physician, ancillary healthcare staff and community agencies in an effort to coordinate appropriate discharge plans from the Medical Center. Case Management also assists patients and families in coping with psychosocial issues that relate to illness or hospitalization. Call extension 184 to contact our case manager. After hours, please contact the nursing supervisor.

Information on advanced directives will be furnished at time of admission.

Surgical Services:
Modern surgical procedures performed by Surgical Associates of South Alabama, PC at Evergreen Medical Center include, but are not limited to: ENT, gynecology, vascular, urology, pediatric, and oral.

The physicians from Surgical Associates hold clinic and perform surgery on Tuesdays, Wednesday, and Fridays. For more information about our surgical services, please call 578-2273.

Volunteers:
The Sunshine Volunteers contribute many hours of service to the hospital. Through the collection of the television usage fees, they have supplied the Medical Center with cots and recliners in the patient rooms, with furniture in the entrance waiting area, and they have also refurbished our chapel. For more information about volunteer opportunities at Evergreen Medical Center, please contact the Director of Marketing at extension 220.


YOUR HOSPITAL BILL, INSURANCE AND PAYMENT

Your Hospital Bill:
Your hospital bill will include the daily service rate and the charge of other services provided. The daily service rate includes the cost of your hospital room, food service, nursing service, housekeeping service and maintenance. Patients are expected to pay out-of-pocket for the television rental charge and visitor food trays.

Payment Arrangements:
Payment arrangements for your hospital bill should be made prior to admission. Should you anticipate any difficulty in financing your stay or have any questions regarding your bill or insurance coverage, please contact our Business Office at extension 169. Business Office hours are 8 a.m. to 4:30 p.m. Monday through Friday.

Insurance:
If you have hospitalization insurance, the Medical Center will bill and accept payment from your insurance company provided that certain conditions are met. However, you are ultimately responsible for payment of your hospital bill regardless of insurance coverage. In the event that the insurance payment is not made within 45 to 60 days, you will be billed for payment in full. Any non-covered charges, such as deductible, telephone, television or any portion of your bill not covered by insurance, will be due at the time of discharge. If you have any questions about your insurance during your stay with us, please call the Business Office at extension 169.

Physician Fees:
Physician fees are not included on your hospital bill. Federal regulations require patients to be billed separately for the technical (medical center) and professional (physician) components of diagnostic studies, anesthesia services, and emergency room physician services.


GOING HOME

Discharge Instructions:

Your doctor and your nurse will give you instructions about post-hospital care. If you have questions about your diet, activities or other matters, please be sure to ask at this time.

Personal Belongings:
Collect all of your belongings and double-check drawers and closets. If you have any valuables stored with the Nurse Supervisor, call the Nurse’s Station and they will be delivered to you.

Escort Service:
When you are ready to leave, a member of the Medical Center staff will escort you to the front entrance and help you into your vehicle.

Evergreen Home Care is a Medicare-certified home care agency which supplies registered nurses, certified nursing assistants, and physical therapists for home health care in Conecuh and surrounding counties. For more information, please call 251-578-6800.
